It provides consent, security, authentication, curation, storage and integration with systems of records. On top of the platform, digital services have been developed to address specific conditions providing; a patient web app to capture Patient Reported Outcome and Experience Measures (PROMS, PREMS), provide messaging and rehabilitation support; a clinical portal to allow interpretation of the data, and improved management and care planning. In addition AI models are applied to the datasets to predict events and provide proactive care planning. The platform has been used for the digital management of diagnosis (Heart Failure) and ongoing monitoring COPD (Chronic Obstructive Pulmonary Disease).

The INCAREHEART PCP project brings together five procurers who want to radically improve integrated care for patients with Chronic Heart Failure (CHF). They are looking for an innovative integrated care solution that overcome fragmented health and care service provision.
