Description:
A Scottish based digital health SME working with the National Health Service (NHS) in Scotland has developed a specialist patient self-reporting mobile app and specialist clinician dashboard for Crohn´s disease and colitis patients.

This has enabled new and more efficient care pathways to be created including the ability to support a patient remotely including remote clinics, system recording, registry functionality and better advance diagnosis.

The service is hosted within the NHS infrastructure and on the companies own cloud based servers.The digital health application suite that they have developed can be used to support a variety of other long term medical conditions. The system can be used by patients to report their symptoms to their clinical care team wherever they are based.

The patient has a remote consultation with the care team using either phone or video. All the symptoms they report and information given are logged on the system with additional longer term information collected and analysed by the application from clinical systems so that patients receive specific advice based on their interaction with the care team.

Healthcare professionals can record medical information in real time on the application console. A clinical dashboard graphically displays patients´ symptoms over time. The treatment summary care plan is sent to the providers´ electronic patient record and can also be sent to primary care if the patients require to stay in hospital.

The system also has data registry including export to central registries functionality and key performance indicator functions.The system is now being rolled out to further NHS areas in the United Kingdom and has been subject to an academic trial in Ireland.

The company are keen to work with private medical organisations and public healthcare providers in order to improve the technology and collaborate to make functional improvements to suit the location and requirements of different regions and conditions.

They are looking for partners to help validate and develop their software applications and potentially look at supporting health conditions that the system has not been used on before. This would be with a view to a potential research agreement.

The technology innovation developed is the system integration between the patient and the health care provider both in terms of managing the communication path, data capture and data analysis on one overall platform which can then feed into larger health care systems. Currently, no similar system with this level of integration has been discovered by the company in terms of healthcare. The system enables patients to have much better understanding of their condition and how they can take control through access to directly relevant information and decision support. Patients feel that they are better supported through access to knowledgeable people and cross organisational contacts who can help with concerns and needs when required.
